Fees, Terms, Waiver and Conditions
I am the parent or guardian of the below named player. In consideration of the player’s participation in the program, I hereby release and indemnify The Greenwich Stars, Raybin Management, LLC, John Raybin, YMCA of Greenwich, YWCA Greenwich, Greenwich Public Schools, The Boys and Girls Club of Greenwich, Camp Simmons, Bendheim Western Greenwich Civic Center, Whitby School, The Leffell School, all facilities used for programming and their owners, operators, directors, Board of Trustees, Board of Directors, officers, administration, employees, agents, managers, staff, volunteers, family members, teachers and coaches against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the program.
By agreeing to this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that the below named player may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at a Greenwich Stars (Raybin Management, LLC) program may result from the actions, omissions, or negligence of the below named player and/or coach, staff member, volunteer and any and all people associated with this program. In consideration of the player’s participation in the programs, I hereby release and indemnify The Greenwich Stars, Raybin Management, LLC, John Raybin, YMCA of Greenwich, YWCA Greenwich, Greenwich Public Schools, The Boys and Girls Club of Greenwich, Camp Simmons, Bendheim Western Greenwich Civic Center, Whitby School, The Leffell School, all facilities used for programming and their owners, operators, directors, Board of Trustees, Board of Directors, officers, administration, employees, agents, managers, staff, volunteers, family members, teachers and coaches against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the program.
There are no refunds, returns or exchanges. A prorated credit for future Greenwich Stars programming will be issued only if the season is paused, delayed or does not occur. Disruptive behavior that leads to the dismissal of a player is at the sole discretion of Raybin Management, LLC (Greenwich Stars) and there will be no refunds, returns, credit or exchanges.
I hereby grant the The Greenwich Stars, Raybin Management, LLC, John Raybin, affiliated partners, the right to use the player’s name, image or likeness in all digital, electronic, broadcast and printed material.
