Fees, Terms, Waiver, & Conditions

I am the parent or guardian of the above named registered player. In consideration of the player’s participation in the program, I hereby release and indemnify The Greenwich Stars, Raybin Management, LLC, John Raybin, YMCA of Greenwich, YWCA Greenwich, Bendheim Western Greenwich Civic Center, The Boys and Girls Club of Greenwich, Greenwich, CT Public Schools, Camp Simmons, The Leffell School and their directors, officers, employees, agents, staff, volunteers, family members, teachers and coaches, as well all directors, owners, administration, Board of Trustees, Board of Directors, managers, staff and operators of all facilities used for all programs from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the programs.

By agreeing to this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that the above named player may be exposed to or infected by COVID-19 by participation; and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at a Greenwich Stars (Raybin Management, LLC) program may result from the actions, omissions, or negligence of the above named player and/or coach, staff member, volunteer and any and all people associated with this program. In consideration of the player’s participation in the programs, I hereby release and indemnify The Greenwich Stars, Raybin Management, LLC, John Raybin, YMCA of Greenwich, YWCA Greenwich, Bendheim Western Greenwich Civic Center, The Boys and Girls Club of Greenwich, Greenwich, CT Public Schools, Camp Simmons, The Leffell School and their directors, officers, employees, agents, staff, volunteers, family members, teachers and coaches, as well all owners, administration, Board of Trustees, Board of Directors, managers, staff and operators of all facilities used for all programs from and against all claims, liabilities, damages or causes of action arising out of or in connection with the player’s participation in the program.

There are no refunds, returns or exchanges. A prorated credit for future Greenwich Stars programming will be issued only if the Greenwich Star Program is paused, delayed or does not occur. Disruptive behavior that leads to the dismissal of a player is at the sole discretion of Raybin Management, LLC (Greenwich Stars) and there will be no refunds, returns, credit or exchanges.

I hereby grant the The Greenwich Stars, Raybin Management, LLC, John Raybin, affiliated partners, the right to use the player’s name, image or likeness in all digital, electronic, broadcast and printed material.
